| Team Handbook | This extensive handbook tells you all that you need to know about MOE. It includes the application, contract, H-H agreement, and the sponsor letter which is also listed as a separate document below. |
| Student Application (closed for 2010-2011) | The application is required to be considered for the team. Application will be updated each summer for the following school year and will be available here. |
| Event schedule, payment, and Participation forms | Important details for students, parents, and mentors. Form will be individually sent out to team members each year. |
| Medical Consent Form | Required for full-team members. Contains consent for emergency medical authorization. |
| Student Contract | Required for full-team members. Students agree to terms and conditions of being a team member. |
| Team 365 Hold-Harmless Agreement 2011-2012 | Required for full-team members, both students and adults. Members agree to accept liability. |
| Student Travel Permission Slip 2011-2012 | Required for students traveling to Regional or National competitions. Gives permission from parents and school officials for time off of school to compete with team. |
| Sponsor Letter - Businesses | For potential sponsors -- provides information for businesses about the team and informs sponsors of the benefits and reasons for sponsorship. |
